Application for Grader and Teaching Assistantships (Fall 2013)

STARTING A NEW JOB (T.A., R.A., Grader, Proctor)

  1. Download part-time job application (MS Word version) (PDF version).
  2. Use the sample form as a guide for filling in the information. If you make any errors, there may be delays in processing the form, and you may have to meet personally with Business Office staff.
  3. Complete the form, preferably on the computer or print by hand. If you fill in the form by hand, please ensure that your writing is legible. Use blue or black only.
  4. Print out the form.
  5. Sign the form by hand and also get original signatures from the job's supervisor.
  6. Drop the form in the box outside Shelley Segal's office, 208 GSIA. Alternatively, you can give the form to Jean Xu, within the same office suite.
  7. You are done. The Business Office may contact you for any corrections or clarifications.

    STARTING A NEW UNDERGRAD RECITATION JOB
  1. Download recitation job application (MS Word version) (PDF version).
  2. Use the sample form as a guide for filling in the information. If you make any errors, there may be delays in processing the form, and you may have to meet personally with Business Office staff.
  3. Complete the form, preferably on the computer or print by hand. If you fill in the form by hand, please ensure that your writing is legible. Use blue or black only.
  4. Print out the form.
  5. Sign the form by hand and also get original signatures from the job's supervisor.

**All students who are hired to work at Carnegie Mellon University must complete and submit an I-9 "Employment Eligibility Verification" form when hired for their first job.**

  1. Students should go to the University Payroll office, UTDC building, Henry Street and ask to complete an I-9 form.
  2. When you go, you should take the appropriate documents from Lists A, B and/or C, which are required for this verification process.
  3. Students should ask for a photocopy of the completed I-9 form to bring back to the Tepper Business Office (Shelley Segal, 208 GSIA, or drop box in the vicinity of this office).


OBTAINING A SOCIAL SECURITY NUMBER (SSN)

International students become eligible to apply for the US Social Security Number (SSN) when they are hired for their first job on campus. In order to apply, you must complete:

**The OIE SSN form must be signed by either Lawrence Rapp or Shelley Segal.**

Once these two forms are completed, students should follow the instructions provided by OIE.

REGISTERING FOR iPAY

Follow the instructions for Registering for iPay. Once you are registered, you will be able to see all of your pay statements, including year-end W-2 forms.
